EQUIREMENTS
.--A comprehensive work-

place wellness program shall be made available by an
4
eligible employer to all employees and include the fol-
5
lowing components:

(A) Health awareness initiatives (including
7
health education, preventive screenings, and
8
health risk assessments).
9
(B) Efforts to maximize employee engage-
10
ment (including mechanisms to encourage em-
11
ployee participation).
12
(C) Initiatives to change unhealthy behav-
13
iors and lifestyle choices (including counseling,
14
seminars, online programs, and self-help mate-
15
rials).
16
(D) Supportive environment efforts (includ-
17
ing workplace policies to encourage healthy life-
18
styles, healthy eating, increased physical activ-
19
ity, and improved mental health).

PPLICATION
.--An eligible employer desiring to
21
participate in the grant program under this section shall
22
submit an application to the Secretary, in such manner
23
and containing such information as the Secretary may re-
24
quire, which shall include a proposal for a comprehensive
