PPLICATION
.--The State submits an appli-

cation to the Secretary that includes, in addition to
2
such other information as the Secretary shall re-
3
quire--
4
(A) a proposed budget that details the
5
State's plan to expand and diversify medical as-
6
sistance for non-institutionally-based long-term
7
services and supports described in subsection
8
(f)(1)(B) under the State Medicaid program dur-
9
ing the balancing incentive period and achieve
10
the target spending percentage applicable to the
11
State under paragraph (2), including through
12
structural changes to how the State furnishes
13
such assistance, such as through the establish-
14
ment of a ``no wrong door--single entry point
15
system'', optional presumptive eligibility, case
16
management services, and the use of core stand-
17
ardized assessment instruments, and that in-
18
cludes a description of the new or expanded of-
19
ferings of such services that the State will pro-
20
vide and the projected costs of such services; and
21
(B) in the case of a State that proposes to
22
expand the provision of home and community-
23
based services under its State Medicaid program

through a State plan amendment under section
