``(i) the goals of the program to be imple-

mented or renewed under the demonstration
2
project;
3
``(ii) the expected State and Federal costs
4
and coverage projections of the demonstration
5
project; and
6
``(iii) the specific plans of the State to en-
7
sure that the demonstration project will be in
8
compliance with title XIX or XXI;
9
``(C) a process for providing public notice and
10
comment after the application is received by the Sec-
11
retary, that is sufficient to ensure a meaningful level
12
of public input;
13
``(D) a process for the submission to the Sec-
14
retary of periodic reports by the State concerning the
15
implementation of the demonstration project; and
16
``(E) a process for the periodic evaluation by the
17
Secretary of the demonstration project.
18
``(3) The Secretary shall annually report to Congress
19
concerning actions taken by the Secretary with respect to
20
applications for demonstration projects under this section.''.

(j) Subtitle F of title III of this Act is amended by
22
adding at the end the following:
