LIGIBLE ENTITY
.--The term `eligible enti-

ty' means a qualified community mental health pro-

gram defined under section 1913(b)(1).

PECIAL POPULATIONS
.--The term `special

populations' means adults with mental illnesses who
5
have co-occurring primary care conditions and chron-
6
ic diseases.

``(b) P
ROGRAM
A
UTHORIZED
.--The Secretary, acting
8
through the Administrator shall award grants and coopera-
9
tive agreements to eligible entities to establish demonstra-
10
tion projects for the provision of coordinated and integrated
11
services to special populations through the co-location of
12
primary and specialty care services in community-based
13
mental and behavioral health settings.

``(c) A
PPLICATION
.--To be eligible to receive a grant
15
or cooperative agreement under this section, an eligible enti-
16
ty shall submit an application to the Administrator at such
17
time, in such manner, and accompanied by such informa-
18
tion as the Administrator may require, including a descrip-
19
tion of partnerships, or other arrangements with local pri-
20
mary care providers, including community health centers,
21
to provide services to special populations.
