(1) I
N GENERAL
.--An applicant for enrollment
3
in a qualified health plan offered through an Ex-
4
change in the individual market shall provide--
5
(A) the name, address, and date of birth of
6
each individual who is to be covered by the plan
7
(in this subsection referred to as an ``enrollee'');
8
and
9
(B) the information required by any of the
10
following paragraphs that is applicable to an en-
11
rollee.

(2) C
ITIZENSHIP OR IMMIGRATION STATUS
.--The
13
following information shall be provided with respect
14
to every enrollee:
15
(A) In the case of an enrollee whose eligi-
16
bility is based on an attestation of citizenship of
17
the enrollee, the enrollee's social security number.
18
(B) In the case of an individual whose eligi-
19
bility is based on an attestation of the enrollee's
20
immigration status, the enrollee's social security
21
number (if applicable) and such identifying in-
22
formation with respect to the enrollee's immigra-
23
tion status as the Secretary, after consultation
